Researchers at the University of Trento in Italy offer new hope for cystic fibrosis patients through an mRNA-based gene editing technology capable of permanently correcting a rare genetic mutation, paving the way for treating the demographic that does not respond to current medications.

Cystic fibrosis is a serious genetic disorder affecting the lives of more than 100,000 people worldwide. Despite significant medical progress, a subset of patients still lacks effective treatment options. In a promising scientific breakthrough, scientists at the University of Trento have developed an innovative treatment based on gene editing technology that can permanently correct a genetic mutation responsible for a severe form of the disease. This discovery provides the first potential treatment for about 10% of patients who currently have no approved drug options.

What is cystic fibrosis and how does it threaten patients’ lives?

Cystic fibrosis is a genetic disorder that causes severe damage to the lungs, digestive system, and other organs in the body. The disease results from mutations in the gene responsible for producing a specific protein that plays a vital role in transporting chloride and bicarbonate ions across lung cells, helping regulate mucus hydration. When this protein is absent or malfunctions, thick, sticky mucus accumulates in the lungs, leading to blocked airways, an increased risk of recurrent bacterial infections, and progressive lung damage, which is the leading cause of death among cystic fibrosis patients.

Replacing a single letter to correct the genetic flaw

Details of this pioneering study were published on April 22 in the prestigious scientific journal «Science Translational Medicine», where researchers described a new base editing strategy aimed at repairing a specific mutation known as (1717-1G>A) in the disease-causing gene. This complex mutation prevents the production of the protein necessary for proper lung function.

The scientific team used a highly precise tool based on a modified «CRISPR-Cas9» system paired with an adenine base editor. They delivered this treatment to cells using messenger RNA technology, allowing the gene editing tool to replace a single disease-causing adenine base with a guanine base in the DNA sequence. In this context, researcher «Anna Cereseto» from the Department of Cellular, Computational and Integrative Biology at the University of Trento, who co-led the research alongside researcher «Alessandro Umbach», stated: “We are repairing that single defective building block within the gene to restore the production of the protein that regulates gas exchange and immune defense in the lungs.”

Promising results in preclinical trials

This new therapeutic approach achieved a gene editing efficiency of up to 30% in human embryonic kidney cell lines and nearly 13% in bronchial epithelial cells extracted from patients, with very limited off-target side effects recorded. Previous research has indicated that achieving an efficiency rate of just 10% may be sufficient to restore the normal function of the missing protein channels.

In addition, the team successfully restored protein activity in intestinal organoids derived from cystic fibrosis patients, a work carried out in collaboration with KU Leuven in Belgium. Pulmonary epithelial cell tests were also conducted in collaboration with the Telethon Institute of Genetics and Medicine in Naples, Italy.

Advantages of using messenger RNA

One of the standout strengths of this treatment is its delivery method. Because the gene editing tool is introduced into the body as messenger RNA rather than traditional DNA, it makes a permanent correction to the human genome and then degrades and fades from the body naturally. This mechanism helps minimize the risks of prolonged unwanted genetic activity, significantly raising the safety level of the treatment.

The biggest challenge: overcoming the pulmonary mucus barrier

Despite these remarkable results, the treatment is still in its preclinical stages. Researchers acknowledged that the primary hurdle facing them now is finding an effective way to deliver the gene editing tool to lung tissue in living patients. Anna Cereseto explained that the bottleneck lies in devising systems capable of penetrating dense mucus and delivering the treatment to the lung epithelium.

These efforts gain twofold importance when looking at the reality of available treatments today. Despite the major success of protein-modulating drugs in recent years, which have transformed many lives, they require at least a trace amount of defective protein to be present in order to improve its function. For patients carrying mutations that lead to the complete absence of protein production, such as the mutation studied in this research, these drugs are useless, making gene therapy the radical and only option capable of correcting the course of the disease from its roots.

To overcome this challenge, the scientific team aims to develop an inhalable delivery system through a strategic project supported by the Italian Cystic Fibrosis Research Foundation. With patients continuing to suffer worldwide, this research represents a genuine beacon of hope toward finding a curative and definitive treatment that restores their ability to breathe freely.

Frequently Asked Questions

What gene editing technology is used to treat cystic fibrosis in this study?

The technology relies on a modified base editing system that replaces a single defective letter in the DNA (adenine with guanine) to restore cells’ ability to produce the missing protein.

What percentage of patients will benefit from this new treatment?

This treatment primarily aims to help about 10% of cystic fibrosis patients who carry specific mutations and do not respond to currently available medications.

What is the biggest challenge facing the application of this treatment in humans?

The biggest challenge currently is devising an effective method to deliver the gene therapy to lung tissue to penetrate thick mucus layers, and researchers are working on developing a dedicated inhalation system to overcome this obstacle.
